Phone Consultation

Our phone consultation is an opportunity to answer your questions and determine what type of service is best for you. Not quite sure what health and aging needs you might have? Not a problem! Over our phone call we will assess your current situation, prioritize areas of concern, and make recommendations for next steps. If The Care Plan is not the right answer for you, we will make referrals to the appropriate agencies and stay in communication to be sure your needs were met.

Determine Applicable Services

During the phone consultation, our Navigation Manager will work with you to understand if your needs are best served by The Care Plan. We offer two (2) main service categories: Care Consultation & Management to plan for immediate or more urgent health and aging needs and My Care Map, a comprehensive package to guide you through each phase of aging.

Care Consultation & Management

We also address your more urgent health and aging needs on a case-by-case basis. With Care & Consultation Management your Navigation Manager will simultaneously assist you with immediate needs and provide you with information on how to stabilize your health.  Learn More >>

After determining the service type right for you, we confirm pricing and set a date for an In-Home Consultation. We see clients primarily in the Chicagoland area, however a virtual phone consultation, or travel arrangements can be made to accommodate your needs.

Payment & Pricing

At this time Medicare, Medicaid and health insurance do not pay for The Care Plan services, all service provided is private pay. Payment for the initial consultation is due at the time of service, additional service hours are billed on a bi-monthly schedule and cash or check are acceptable forms of payment. Additionally, we have a deep commitment to the development of affirming care and resources for trans-identified and gender spectrum clients. Our services are provided on a sliding scale to need-based individuals, please call for more information.

In-Home Consultation

We Come to You

A Navigation Manager is prepared to meet with you in the comfort of your home (or senior living facility, rehabilitation homes, or hospital) at your convenience. The duration and composition of the In Home Consultation will vary depending on the service provided. For all services, please be prepared to share medical history and profile, provider information (insurance, physicians, treatment providers), relevant advanced directives, employment and social history. Prior to the In-Home Consultation, you will be given a checklist of materials to prepare and questions to consider.

Health & Aging Assessment

At The Care Plan, we believe no one understands their body and needs more than themselves. Our first step is to have an in-depth conversation with you regarding specific health and aging concerns, the interventions we observe and recommend, and what steps should be taken. Our In-Home Consultation also includes a Safety Assessment to identify areas for improved safety within your environment.


Care Consultation

For any Care Consultation & Management services, family, friends and members of the support system are welcome, however one-on-one time between the Navigation Manager and person who requires care will be required. We ask that no more than five (5) people be present for Care Consultation & Management sessions, and no more than three (3) for My Care Map consultations. Please notify your Navigation Manager ahead of time if you would like to request an exception.

Documentation Creation & Research

Following the In-Home Consultation, a tentative date for the Delivery Consultation will be set, ideally within one (1) to two (2) weeks. Meanwhile, your Navigation Manager will get to work drafting documentation and compiling relevant resources and recommendations related to your care. In the case of My Care Map, a rough draft will be submitted for your review prior to the Delivery Consultation to provide you with a chance to review and make changes, if necessary.

Delivery Consultation

The same Navigation Manager you met with during the In-Home Consultation will conduct your Delivery Consultation. He or she will also be the one to follow up with you and coordinate any future service needs. He or she will also share with you findings, and provide documentation tailored to your situation.

My Care Map

If you elected My Care Map as a service, the Navigation Manager will issue your paper copy of the final document and send you an electronic version. We take a big picture approach to care, and My Care Map will contain recommendations for when to follow-up and a timeline to support the achievement of your long-term goals. Your Navigation Manager will review the document with you and up to three (3) loved ones of your choosing, answer questions and facilitate the discussion amongst those present.

Care Consultation & Management

The Care Consultation & Management services often require action of a more immediate nature. We understand the urgency in health and are prepared to address your needs whenever they may arise during our process. Health needs fluctuate so we are prepared to work on immediate tasks and future goals simultaneously. For example, if you would like your Navigation Manager to advocate for you at a physician appointment prior to your Delivery Consultation date, we can certainly support you in this manner. Typical services include: health maintenance, consultation & advocacy, caregiver training, and LGBTQI support. For a full list of services please visit the Care Consultation & Management page.


Following the Delivery Consultation, you and your support team should act on The Care Plan team recommendations. Your Navigation Manager can work with you or your loved one on an ongoing basis to complete tasks and set you up for a successful health and aging experience. Life can be overwhelming at the best of times, and when you find yourself in the midst of a health crisis, it can feel disempowering.

Our Care Consultation & Management services are designed to support as much or as little as you prefer, always keeping you in the driver’s seat.
